1. Page renamed to Request Pricing. Every question on the live form is a quoting question, so the page and nav label now promise pricing, lead times, samples, and specs instead of "we would like to hear from you."
2. Four intent doors up top. Price a project, get a catalog, find my rep, or call. Three different audiences stop sharing one 19-field form.
3. Form cut from 12 required fields to 6. Line, name, company, email, and state are required. Fax is gone, comments and postal code are optional.
4. Two steps, with the second optional. A visitor can send after step one, or add project detail for a tighter number. "Just have my rep contact me" skips step two entirely.
5. Line selection drives the form. It is the routing key, so it comes first and changes downstream labels, including rooms versus beds.
6. Catalog request folded in. The separate catalog form duplicated this project block. One intake with a needs multi-select gives cleaner CRM data and one workflow.
7. Rep routing shown live. Pick a state and the matched rep appears before submitting, so no one wonders where the request went.
8. Two duplicate rep tables became one lookup. Same reps were printed twice. State plus line now returns a contact card with tappable email and phone, and unassigned states fall back to the sales manager instead of showing nothing.
9. 50 red state chips removed. They competed with the form and broke badly on mobile, where emails ran off screen and phone numbers split mid-number.
10. Expectation setting before submit. Who replies, how fast, and no obligation, next to the send button.
11. Data fix. The live Alabama hospitality row lists 615-714-84077, an extra digit. Corrected here to 615-714-8407.
